You'd need to find a temple or a school (if you wanted to be affiliated with a specific branch of Buddhism) and basically what you would do is take refuge in the three treasures: The Buddha as your teacher, the Dharma as your hand book for life, and the Sangha the community of believers. Taking refuge will make you a Buddhist, but there is (at least in my sect) a ceremony called Jukai which means that you are becoming a Nichiren-Shu Buddhist. There might be something like this with Tibetan Buddhists... which is why you would really need to find a teacher in this order if this is what you want to do.
